March 5, 2019 ---- Full Time, Title XIX Waiver Program Cooridnator

Appalachian Community Health Center is now recuiting for a Title XIX Waiver Program Coordinator. A person in this supervisory position will be responsible for all aspects of the Home-Based and Community Waiver program in the Center's catchment area of Randolph, Upshur, Barbour and Tucker counties. In addition, a person in this position will work with Center and other supervisors to support the mission of the Center and provide services to the ID/D population. A successful canidate will have a minimum of a bachelor's degree in a human services field plus at least 2 years of working with this population, a master's degree in a human services field plus experience with the population or a registered nurse. Attenion to detail, ability to work with the ID/D population, ability to work with staff and cargivers and the ability to work with other agenciers will be required.

Position will require home visits as well as other serivce coordination functions and will work as part of a team in providing the serivces. A minimum of a bachelor's degree in a human services field is required and experience working with this population is a plus.

February 5, 2019 ---- Full Time, Children's Mobile Crisis Specialist

Appalachian Community Health Center is implementing a new program and is currently requiting for a Children's Mobile Crisis Specialist to be responsible for implementing and day to day operations of this program. A person in this position will provide emergency services and crisis intervention to children and youth in Randolph, Upshur, Barbour and Tucker counties and may be provided in a variety of locations and will include crisis support and diversion services. In addition, a person in this position will work closely with other community agencies as well as other Center programs. Position requires a minimum of a Bachelor's degreee but a Master's degree in a human services field is preferred. Two years of experience working with children and youth is required and experience providing crisis response and interventions is a plus. Travel throughout the four counties will be required. Appalachian Community Health Center is currently recruting for a Service Coordinator to provide case management services to a variety of consumers with behavioral health needs. A person is this position works closely with consumers and others including other staff to provide the best services possible to those individuals who have mental illness, substance use or intellectual disabilities. A Bachelor's degree in a human services field is required, experience providing case management services is a plus. Position will be in the Elkins Office.
