Appalachian Community Health Center is recruting for an IDDW Case Manager (CM). The position is a full-time position,
40 hour work week, Monday through Friday. Accrual of annual, sick leave benefits after first month of work.
CM will assist and enable individuals/participant with cognitive and or developmental
disabilities to remain in the least resstrictive environment while maintaining the greatest
amount of independence and human dignity. IDDW Case Manager assists the
participant with access to and coordination of home and communithy-based services to
enable the individual to receive services in a planned, coordinated, efficient and
effecive manner. CM will promote and support the participants preferences, values, and
the right to self-determination. The CM evaluates ongoing services as well as members
response to treatment/services plans.
*The Minimum of a Bachelor's degree from an accredited institution, in a human
Services field such as Psychology, Social Work, Education, Nursing or related.
Appalachian Community Health Center is recruting for a Master's level Therapist to provde therapy services
to individuals and families in Randolph, Upshur, Barbour and Tucker County areas. This position can be
full-time and competitive benefit package including health insurance, paid time off, and can be part-time or contractual.
A minimum of a Master's degree in a Human Services field and licensed preferred but license eligible will be considered.
Experience working with adults and children is preferred but licensed eligible will be considered. Experiecne working with
adults and children is preferred. Must be able to pass a background check and experience working with the substance using
populaiton is a plus.
